I hereby authorize Zero To Three, National Women’s Law Center, Pritzker Children’s Initiative, and the Robert Wood Johnson Foundation (collectively, the “Story Bank Organizations”) to reproduce, distribute, edit (provided the substance of my story remains intact), publicly display, and otherwise use the material I submit to the Story Bank (“my Story Bank Submission”) in any of their materials that advances the missions and goals of the Story Bank Organizations, including, but not limited to, on websites, in social media posts, in op-eds, in government testimony, in publications, presentations, advertising, and in pitches to reporters. In all instances, the Story Bank Organizers may use my first name and my state, as well as the first names of my family members and others about whom I write in my Story Bank Submission. The Story Bank Organizations may not disclose my full name or my contact information to third parties without my express consent.
I waive all right to inspect or approve of the use of my Story Bank Submission by the Story Bank Organizations, now or in the future.
I understand and agree that I will receive no monetary compensation in exchange for the use of my Story Bank Submission by the Story Bank Organizations.
I release and discharge any and all actions or claims which I, my family members, or my heirs may have against the Story Bank Organizations, their officers, directors, trustees, employees, contractors and/or agents, arising for any reason whatsoever from any distribution, publication, or use of my Story Bank Submission in accordance with this Consent Agreement at any time now or in the future. I agree to defend, indemnify, and hold harmless the Story Bank Organizations, and their officers, directors, trustees, employees, contractors, professional advisors, assignees and agents, from and against any and all claims, demands, expenses, losses or liabilities including, without limitation, reasonable attorney’s fees, arising out of or in connection with any breach, or alleged breach, of the terms of this Consent Agreement.
I represent and warrant that the facts set forth in my Story Bank Submission are true. I further represent and warrant that I have reached the age of majority in my jurisdiction and understand and agree to the terms set forth herein.
